Our Progress

It is within our power as a community to create systems and environments that make it easier for all of us to live healthier lives. A community health improvement plan acts as a roadmap for how partners will work together to counter preventable health problems and improve community well-being over the long term.  See how we're progressing on the road to a healthier SLO County.

Healthy Neighborhoods

Socially cohesive, vibrant neighborhoods allow people to connect with each other, safely walk and bike, access healthy food and other things they need, and enjoy parks and open space.

Partners: 805 La Voz, Adult Services Policy Council, Caltrans, First 5 SLO County, Food Systems Coalition, Healthy Communities Work Group, Mujeres de Acción, Public Health Department, Safe Routes to School Taskforce, SLO County Parks, SLOCOG, UndocuSupport

KEY Accomplishments:

​

  • Healthy Communities Work Group has been chosen to be the health team championing this priority area.
     

  • Discussions are underway around a place-based approach to understand high-priority communities and their built environment needs (e.g. complete streets, food access, park access).
     

  • Published collaborative Climate Resilience Study to better quantify and qualify the projected impacts of climate-related hazards, including drought, extreme heat, flooding, and wildfire, on SLO County health.

Access to Care

Partners: Adult Services Policy Council, Care Coordination Coalition, Center for Family Strengthening’s Promotores Collaborative, Public Health Department (PHD), PHD Health Equity Program, SLO County Parks, SLO Healthcare Workforce Partnership, UndocuSupport

Healthy communities ensure individuals can easily get care that is affordable, culturally and linguistically appropriate, and available when and where they need it.

key Accomplishments:

​

  • Published data and market assessment of local healthcare positions (number and type of positions needed) from local healthcare providers and the Workforce Development Board (also shown here).
     

  • Hosted an October 2024 SLO County Healthcare Workforce Forum with 75 partners, representing a multitude of healthcare professions. 
     

  • Created infographic detailing current educational/training/professional development opportunities to build the healthcare workforce.

Mental Health & Substance Use

Partners: Adult Services Policy Council, Behavioral Health Department (BHD), CenCal, Child and Family Wellness Collaborative, Department of Social Services (DSS), EMSA, First 5, Opioid Safety Coalition, Parent Connection, PHD Healthy Aging Services, Public Health Department (PHD), SLO County Parks, Suicide Prevention Council, Transitions-Mental Health Association (TMHA)

Thriving and resilient communities include opportunities for people to have socio-emotional supports that prevent anxiety, depression or substance use from beginning in the first place as well as treatment for those issues when they do arise.

key Accomplishments:

​

  • Formed a Mental Health & Substance Use health team to champion the efforts under this priority area.
     

  • Published fact sheet around suicide deaths that provides key analysis on age, sex, mechanism of injury and veteran status in order to better inform future efforts.
     

  • Published fact sheet around drug-related deaths that provides analysis on age, sex, region and drugs involved in drug-related deaths in order to better inform future efforts.
